Dr. Translational Evolution of Post-COVID Multisystem Disorders: From Long COVID to Immune-Mediated Neuro-Interstitial Syndromes

Post-acute sequelae of SARS-CoV-2 infection [PASC], initially characterized as Long COVID during 2020–2022, have undergone significant conceptual transformation. Early descriptions focused primarily on fatigue, respiratory symptoms, and neurocognitive complaints. However, translational and clinical research from 2023–2026 reveals a broader multisystem disorder involving immune dysregulation, interstitial lung pathology, neuroinflammation, and microvascular injury. Emerging evidence suggests overlap with autoimmune and granulomatous diseases, highlighting a shift from symptom-based classification to mechanistic, phenotype-driven frameworks. This article synthesizes current translational insights into disease evolution, pathophysiology, and clinical stratification of post-COVID syndromes.
